Patient Portal Guide

The patient portal is a secure, online tool that allows you to access your health information and communicate securely with the clinic.

You can use the portal to:

• View, download, and transmit your health information
• Send a new message to the clinic or reply to a message sent by the clinic
• Request a non-urgent appointment or cancel a scheduled appointment
• Request a medication refill
• Send the clinic health data from other doctors or sources (ex: self-recorded blood pressure logs)
• Ask general, non-urgent questions or follow-up questions from a previous visit

Telemedicine Appointments

As a patient of our clinic, we are looking forward to your first Telemedicine visit. You can now use the Telemedicine app to meet with your provider online from the convenience of your own home!

A telemedicine appointment is just like a regular appointment. You will sit in front of a camera where you will be able to see yourself and your health care provider on the screen once your session begins.

Your health care team will ask you questions and work with you to develop a plan of care, providing the same quality of care you would get in a face-to-face visit.

The process is simple and can be done by following these these four steps:
